What is Dowager's hump?
Dowager's hump, or kyphosis, refers to an excessive outward curvature of the upper spine and forward head tilt, leading to a hardened mass at the base of the posterior (back) of the neck. This condition typically impacts the thoracic spine, causing shoulders to slump forward and the head to tilt, creating a forward head posture.
Forward head tilt involves the protrusion of the head in front of the shoulders, contributing to increased strain on the neck and upper back muscles. Prolonged sitting, improper ergonomics, and a lack of regular physical activity can exacerbate this postural issue.
Dowager's hump is often caused by tight pectorals pulling the shoulders forward and weakened rhomboids unable to support the shoulders correctly. Combined with a forward head tilt, tight anterior cervical neck muscles and weakened posterior cervical muscles contribute to this condition.
Symptoms Dowager's hump
1. Visible Hump: The most noticeable symptom is a visible hump or curvature at the base of the neck or upper back.

2. Forward Head Posture: Dowager's hump often leads to a forward head tilt, where the head protrudes in front of the shoulders.

3. Shoulder Slumping: The shoulders tend to slump forward, contributing to the hunched appearance associated with kyphosis.

4. Neck and Back Pain: Individuals with Dowager's hump may experience discomfort, pain, or stiffness in the neck and upper back regions.
​
5. Limited Range of Motion: Reduced flexibility and a limited range of motion in the spine can be indicative of this condition.

6. Muscle Imbalance: Weakness in the rhomboid muscles, which support proper shoulder positioning, and tightness in the pectoral muscles are common contributors.

7. Headache: The forward head tilt can lead to increased strain on the neck muscles, potentially causing headaches.
​
8. Fatigue: Maintaining an incorrect posture for extended periods can result in muscle fatigue and overall tiredness.

Treatment
For Dowager’s Hump, we recommend that the client books in for the Hercules injury package. This will include a full assessment of posture, range of motion, and strength of the neck and shoulders. Depending on the results, a typical treatment will consist of STR, massage, hot stones, and IASTM of pectorals to relieve tension and stress from rolling the shoulders forward. Additionally, we would perform massage, STR, and hot stone massage for the neck SCM with MET, STR, massage, and hot stones of the trapezius, along with ultrasound therapy on the posterior of the neck. Completing the session with reassessment and a personalised rehabilitation plan, including exercises and stretches. The number of sessions may vary depending on severity.
